Yes, i've filled out hundreds if not thousands of them.
It shows only the existence of debt. You'd be better off going for the companies GAAP financial statements as that may have additional detail of the debt and who owns it. With the audited and reviewed financials that i produce for my clients, we will name the creditor in the notes to the financial statements.
Tax returns you are not required to show this detail, only the total debt owed.
